Chronic Disease Management

All our practice nurses are specialised in chronic disease management, and run clinics to cover diabetes, hypertension, coronary heart disease, heart failure and other heart conditions, stroke, asthma and COPD. They are experienced in prevention advice, cardiovascular risk assessment, diagnosis and management of these conditions. Anyone recorded with these conditions will be called in regularly for a review, but anyone can book an appointment if they have concerns or would like advice in any of these areas. 

Practice nurse appointments may be booked in reception. Routine appointments may have up to a 4 week wait, but the practice nurses run a walk-in service Monday-Friday 9.30-11am, where anything more urgent can be dealt with. Also, there is always a nurse available for telephone advice Monday-Friday 9-9.30 – please ask reception for a telephone consultation. 

The HCAs are an integral part of the team, carrying out many of the blood and urine tests required for monitoring of long term conditions. They also have appointments available for weight monitoring and stop smoking advice. They do the new patient checks and many of the blood pressure reviews. Additionally Connie is trained in doing spirometry tests. 

HCA appointments may be booked in reception. Routine appointments may have up to a 2 week wait, but there are walk-in blood appointments Monday-Friday 11-12.00.
